Horror fest Covid Protocol

1. I have not knowingly been exposed, nor tested positive, to COVID-19 within the past 14 days. To my knowledge I have not experienced the onset of any primary COVID-19 symptoms within the past 14 days, including: a fever of 100.4 degrees F or higher, a new persistent cough, a shortness of breath or any new breathing difficulties. 

2. For everyone’s safety, Please wear a face covering/mask inside any Convention venue unless actively eating or drinking or during photo ops. 

3. For everyone’s safety, I understand that physical distancing of six feet will be observed at all times in the Convention venue.

4. If I contract COVID-19, and/or experience any COVID-19 symptoms and/or becomes ill while attending the Convention in Phoenix, Arizona during the Convention dates, 

I release from liability and indemnify Artists Assemble Productions and the Sheraton Crescent Hotel.

5. Per Arizona Law, Masks & Vaccination cards are not required but highly  recommended.
